• Right to Access & Copy: You can request your health records in paper or electronic form. We must respond within 30 days (with one possible 30-day extension) and may charge only reasonable fees.

• Right to Request Restrictions: Want certain info kept private or communications via a specific method? We’ll do our best—though we may not always agree if it impairs your care.

• Right to Confidential Communications: Prefer email over mail, or vice versa? Let us know how to reach you most comfortably and securely.

• Right to an Accounting of Disclosures: Curious who else has seen your info? You can request a record of disclosures (excluding common uses like treatment) from the last six years.

• Right to File a Complaint: If you believe your rights were violated, you can either contact our Privacy Officer or file directly with the U.S. Department of Health and Human Services—no retaliation, ever.

We use strong administrative, technical, and physical safeguards to keep your PHI protected—encrypted systems, limited access, secure training for our team.

In the unlikely event of a breach of unsecured PHI, we'll notify you immediately, following all legal requirements.

If we update this Notice, we’ll let you know by posting it on our website and making a paper copy available at any time. The revised notice applies to both current and future information about you.
